Your First Visit!
One of the first ways we get to know you and your pet is through our “new client” form. Feel free to print and complete this at your own convenience and bring it with you to your pet’s appointment, or submit the form online. As a new client, we also ask that you fill out our medical release form. You can print it or complete the online form here so our doctors and staff have it on file at the hospital. Additionally, if you’d like to expedite your pet’s initial screening process, you can fill out our patient history form online. You also have the option to fill out these forms when you arrive to your pet’s visit.
To better understand your pet’s overall health, our team will perform a nose-to-tail physical exam. One of our doctors will listen to your pet’s heart and lungs, check his or her oral cavity, vision and ears, and palpate joints, muscles, lymph nodes, abdomen and skin. Depending upon your pet’s medical history, the veterinarian may also recommend vaccinations, bloodwork, and parasite testing.
